I hereby take responsibility to return the Chromebook(s) in the same condition that I receive them. In lieu of a deposit, I authorize the Hebrew Academy of Cleveland to use my credit card/bank account on file as security. I will be notified with the email address submitted on this form when the deadline will be to return the Chromebooks. If they are not returned by the deadline or if they are damaged (which will be at the Hebrew Academy of Cleveland’s discretion) I authorize the Hebrew Academy of Cleveland to charge my credit card/bank account $150 per device (or $20 per charger) which will be non-refundable.*
